See the JavaScript Guide as well as the Acrobat Help. What you're looking for is "Templates" in the help file, and spawnPageFromTemplate in the JS Guide.
Basically you'll make a template out of the first page, and spawn a new page from it. The spawnPageFromTemplate method will allow you to rename the fields automatically.
