PDF-SnapShot allows a user to capture the image of a form and save it as a PDF file, 
however it  has many other uses,
such as allowing an area of interest within a PDF document to be captured and saved as 
a PDF file.  Adobe Reader 6.0 has
a very useful "snapshot" tool built in - but it won't allow you to save the captured 
image as a PDF.  PDF-SnapShot
lets you save the image directly as a PDF or JPG.  PDF-SnapShot also has automation 
features not present
in either Approval or Reader for automated text and image capturing. The JPG-2-PDF 
utility that comes
with the full licensed version of PDF-SnapShot will batch convert an unlimited number 
of JPG files to PDF.
That would seen to be a good collection of features for $49.
