I've had that happen on the rare occasions my cable gives me a different IPv6 
prefix (the leases being decently long, that's typically a fairly long power 
outage, not just a modem reboot).  Takes awhile to remember that I have 
firewall rules on the router (that sadly cannot be written in terms of the 
assigned prefix).
