Had the same problem with ADSL provider, only those values helped in
pf.conf:
scrub in on pppoe0 max-mss 1400 fragment reassemble
scrub out on pppoe0 max-mss 1400 no-df random-id fragment reassemble
BTW, if you want to set up WWW cache (squid), be ready for more
problems...
