It's good that you found a solution that works for you.

However, what is not good is that there is still a bug in there 
somewhere. Output to default _should_ work and I can't understand why it 
doesn't.

What distribution are you running?
Some distributions like Ubuntu have a Alsa-to-Pulseaudio plugin 
installed by default which is connected to the default device. Could you 
have something like that installed?
