Make sure the HeaderTabs extension is installed: https://www.mediawiki.org/wiki/Extension:Header_Tabs
In the body of your page, use header 1 markup to create the tabs = Tab 1 = Tab 1 content = Tab 2 = Tab 2 content etc. Then include <headertabs/> At the bottom of the last tab’s content. That’s it.
