Windows Server 2008. I have changed it to
$wgHTTPProxy = "http://username:password@<our proxy>:80";
Our debug log does not show it being used and I still cannot use or reference
InstantCommons pages. Any ideas on what might prevent the proxy being used -
or does its use not get logged in the debug log.
Also, in the Debug log I see the line:
Class PEAR_Error not found; skipped loading
but PEAR is installed and is being used for successfully for emails. Any idea
why this might be happening?

You don't mention what OS you're running on, if it is linux, might you try to
set a system environment variable for a http_proxy..?
