I threw together a little plugin for ConfirmEdit that uses questions instead of word-image CAPTCHAs: http://www.mediawiki.org/wiki/Extension:QuestyCaptcha Humans can still solve them, of course, but a person working in an Indian CAPTCHA-cracking farm might have more trouble answering a question specific to your site than just reading a word in an image.
