I use the FancyCaptcha that comes with ConfirmEdit. It needs some setup procedure that can be a little intimidating. The good thing is that it is hard enough to keep all bots away (not a single spam in two years of use). The bad thing is that it is hard enough to keep half of casual anonymous visitors from making edits. There is one usability issue: If the wiki is set to always require confirmation, it is still only show the captcha after the first save try. The captcha should be shown right in the edit page before saving if it is necessary anyways, thus avoiding an additional step in order to save the page. -- Juliano F.
