Either change your layer to say STATUS DEFAULT or add the layer to your URL.  
e.g. 
 
http://localhost:8080/cgi-bin/mapserv.exe?MAP=/ms4w/apache/htdocs/test/test.map&mode=map&layer=Installation
  
 
The status thing catches a lot of people.

 I'm sure there is something here I'm overlooking, but I just can't seem to
