Dear A-Imam

The minimum working examples do not include index entries and do not include 
the  index at the end. Your original file included both.  (By the way, I do not 
see a difference between temp.lyx and temp2.lyx). The MWE does not run on my 
computer because I do not have the picture “Images/Index example.png” you 
included.  Without this picture your MWE runs on my computer.  As your file 
does not seem to invoke any indexing command, I wonder whether the picture is 
the problem. 

I do not now the inner workings of LaTeX/LyX, but have you tried to rename your 
picture?  Sometimes, names with blanks are misinterpreted by programs.

What Jean-Marc meant is that In the LyX menu in the top line of your window 
(Mac: in the top line of the entire screen) there is an entry “View”. If you 
click on it you find a sub-entry “Message Pane”. If you click on that sub-entry 
  LyX will show some kind of text. In my case it reads like that:

In your case you may find some information about the index problem you have.  
(If you do not use the English version of LyX then “View” and “Message Pane” 
will probably be translated into your language)

If the problems persist, even after renaming you image, then you could try to 
explicitly set the index processor in the document settings (In the LyX menu: 
“Document"  ->  “Settings…" -> “Indexes”, the names again refer to the English 
version of LyX). In your example, the processor  is set as “default”.  Other 
choices in my LyX/LaTeX installation are: makeindex, texindy, xindex, xindy.
