Most people do. My understanding is that the license covers System Center "related" databases. WSUS certainly is that. I'd also use it for MDT for example. System Center 2012+ comes with SQL licensing whereby you can install a SQL instance as long as it's used by only System Center products. Does this exclude the WSUS DB (SUSDB)? I asked this question on TechNet a few years ago when I was preparing our environment but didn't get a straight answer so just put it on a separate server.
