    tdf#93739 clarify instructions about inserting form controls
    
      -revise instruction about inserting form controls
      -correct grammatical problem with holding down keys
      -remove obsolete <note> about specifying HTML version
      -update to <note>,<tip>, some <h2>,<menuitem>
      -add <keycode>
      -add missing table id
