To check this, go to View > Header and Footer, and make sure that the "Fields" option is checked. The fields may have been hidden using the "Hidden" option in the Field dialog box. To check this, right-click on the field and select "Field" from the context menu. In the dialog box that appears, make sure the "Hidden" option is not checked.
