The document can be edited just fine in Word for Mac 16.49 (21050901).

However, in Office365 online, the document opens as displayed in the enclosed
screenshot, with the footer image overlying the header image, and seemingly not
being editable. This is due to the "inline" attribute being assigned to the
header and footer when loaded into Word Online.

Word Online only has the following image placement attributes:

Aligned with text (inline)
Left justified
Right justified
Behind text
In front of text

This appears to be a particular behaviour of Word Online, as the on-disk
version of Word for Mac (version 16.49) doesn't show this behaviour and
displays the document correctly when loaded.

FWIW, the online loaded document can be edited, but it is disconcerting for the
user.

What it does show though, is that one can not assume that a document in docx
format which displays normally in the on-disk version of Word will be similarly
displayed in the online version of Word.
