The failure happens rather early in the boot process. when booting Lubuntu 
Questing from a USB pendrive. The grub menu is OK, but the process get stuck 
with a repeated error output to the screen, '/init: line38: can't open 
/dev/sr0: no media
...
Unable to find a medium containing a live image

Obviously I cannot report from the failing system, but I can attach what
was sometime written to the writable partition. (However, I am not sure
if [some of] it was written while booting in another computer, a Dell
Precision M4800, where it works.

Please notice that the same failure occurs when trying to boot Ubuntu
Desktop Questing, so I suspect that this bug affects several if not all
Ubuntu flavours, and I guess the linux kernel is to blame.
