PCIe Multi-Segment support

Today we only support a single PCIe segment, which means we max out at
256 PCIe buses. Some configurations have been closet to hitting this
limitation and we have had to do some reshuffling of bus resources to
make it work. To avoid this on server platforms we need to support PCIe
multi-segment.
