Additional note: Precise guests on the same Xen host don't have problems rebooting, e.g. using the latest linux-image-3.5.0-43-generic kernel.

Our hypervisor is XenServer 6.0 with all the latest patches. If we revert to 2.6.32-48, the problem goes away and reboots are successful.
