The issue persists when using A2DP (as opposed to HSP/HFP), with the following setup:

Switching to HSP always works for me, but the sound quality is terrible. The issue exists on both ubuntu and kubuntu 16.04. I have reported it on the ubuntu support forums and after investigation a moderator noticed changed in the kernel from 4.4 and 4.2 that may becausing the issue and asked me to file a bug report.
