Title: Problem in 4.8.0-22, looks to have been fixed Status in linux package in Ubuntu: Incomplete Bug description: It looks to have been fixed in 4.8.026, but the previous version of the kernel was complaining bitterly, possibly when an SD card reader was unplugged. The first time it happened, two days ago, it was noticed when the partition with /var/log ran out of space thanks to the volume of messages to kern.log and syslog. The second time, yesterday, it was spotted before then: Oct 19 16:05:23 zoo NetworkManager[884]: <info> [1476889523.9804] dhcp4 (eth0): state changed bound -> bound Oct 19 16:18:13 zoo kernel: [80935.249286] ohci-pci 0000:00:12.0: frame counter not updating; disabled Oct 19 16:18:13 zoo kernel: [80935.249291] ohci-pci 0000:00:12.0: HC died; cleaning up Oct 19 16:18:13 zoo kernel: [80935.249469] usb 4-5: USB disconnect, device number 2 Oct 19 16:18:13 zoo kernel: [80935.250560] AMD-Vi: Event logged [IO_PAGE_FAULT device=00:12.0 domain=0x0004 address=0x0000000000000080 flags=0x0020] Oct 19 16:18:13 zoo kernel: [80935.251549] AMD-Vi: Event logged [IO_PAGE_FAULT device=00:12.0 domain=0x0004 address=0x0000000000000080 flags=0x0020] Oct 19 16:18:13 zoo kernel: [80935.252546] AMD-Vi: Event logged [IO_PAGE_FAULT device=00:12.0 domain=0x0004 address=0x0000000000000080 flags=0x0020] Oct 19 16:18:13 zoo kernel: [80935.253547] AMD-Vi: Event logged [IO_PAGE_FAULT device=00:12.0 domain=0x0004 address=0x0000000000000080 flags=0x0020] Oct 19 16:18:13 zoo kernel: [80935.254545] AMD-Vi: Event logged [IO_PAGE_FAULT device=00:12.0 domain=0x0004 address=0x0000000000000080 flags=0x0020] Oct 19 16:18:13 zoo kernel: [80935.255547] AMD-Vi: Event logged [IO_PAGE_FAULT device=00:12.0 domain=0x0004 address=0x0000000000000080 flags=0x0020] .. thousands of similar lines deleted. I can plug in and remove the reader without this happening following today's update. The four requested files are attached, but this is with the new version where it's not happening.
