When the issue happens, I'm disconnected from the AP, the password box pop-ups for the AP, no matter how many times I re-enter the password it will not connect, and the only way I can solve this is to restart the machine. This happened in both 12.04 and 12.10 (I upgraded hoping to fix this issue). You can see ~2m prior there was a successful "rekey", and then the 'No probe response' and after that I lose all connectivity and the password prompt appears.
