I couldn't find any unsafe codepath in the imap resource, so we need some more info first. The imap resource only removes the old item if the append succeeded, so if it fails nothing should happen and the old state should be restored with the next sync. (unless we get an itemRemoved signal from the akonadi server, but I wouldn't see why that should happen).
