The "settings - default applications - file manager" is the same setting as
"Always open Folder files with the chosen app", i.e. the preferred option for
the MIME type inode/directory.
To add an app to "Open with..." without changing the default file manager, you
can either 1) Do what you described, then change the default file manager
setting back to Dolphin; or 2) Add the app in "settings - default applications
- file associations - inode - directory".
