Are you using any screen scaling?
Does the icon stop being blurry if you move the mouse pointer over it?
If you find the file ~/.cache/icon-cache.kcache on disk, and then move it
somewhere else, and then reboot, is the issue fixed now? If so, please attach
the file here.
