I FIXED my issue by also adding `XDG_CURRENT_DESKTOP=KDE` to
`/etc/environment/`.

Now the Dolphin File picker dialog respects my dark theme again and all the
icons are there.

As to why it worked on an older KDE version without `/etc/environment/` - I
don't know.
