This PDF also works in Acrobat, not in Firefox and not at all in Okular.
In Okular clicking on "block calculations" checkbox immeditely triggers a
Javascript error box.

STEPS TO REPRODUCE
1. Open attached PDF
2. click on "block calculations"
3. Immediately triggers javascript error box
OBSERVED RESULT
-> you get a javascript error box
EXPECTED RESULT
-> no error box -> dynamic PDF form can be used just like with acrobat.
