I've seen this recently with only a single screen, but haven't figured out how
to reproduce it. It seems to be related to resolution-related things; sometimes
my plasmashell is blurry and the notifications appear in the wrong place, as
though the resolution is wrong. Restarting plasmashell fixes both issues.
