This happens in kickoff and kicker for me, so i don't think it is specific to
any launcher. Usually after encountering another plasma bug of some sort (like
kwin crashing) and then logging out and logging back in. Kate reappears for me,
and system settings is moved up to the second position, after firefox.
Sometimes there is even a duplicate firefox added to the end of the list, it's
inconsistent for me.
