I can confirm this bug. Conditions:
x86_64 Linux 5.9.13-arch1-1
KDE Frameworks 5.78.0
QT 5.15.2 (built against 5.15.2)

I have attached the output of env. As noted above by Wolfgang Bauer, the bug is
dependent on the value of XDG_CURRENT_DESKTOP, which is to say Okular's idea of
the QT5 configuration. I have tried a few values of this:
Worked; loaded KDE Plasma theme instead of the one I set, and did NOT display
bug:
kde
Didn't work 1; Correctly loaded QT5 theme I set, and displayed bug:
gnome
xfce
mate
x-cinnamon
unity
lxde
Didn't work 2; Didn't load the QT5 theme I set, and displayed bug:
all other values, e.g. 'kde-plasma', 'kde5', 'lightdm', 'i3'
