If the last thing you clicked was the desktop or the plasma panel it works. But if you clicked in another window and immediately after right click in the plasma panel, the add panels sub-memu will appear in another window. And there is another way of triggering this bug that I don't know if is related. If you open konsole and run a command that takes some time.
