I've just tested the PDF from the link and after clicking Show Forms I have all
black boxes but that means they're now editable.
I've ticked a few boxes and typed in a few fields all OK..
Can you please confirm if this issue still persists or if we are able to mark
this bug report as resolved, thanks.
