Very similar symptoms here, on a fresh install of Kubuntu 16.04.
On this occasion, plasma crashed when I clicked on the panel to restore
Firefox. Other crashes happen randomly when the mouse moves over the panel, or
I click on an app icon on the panel. It doesn't happen every time, just
randomly a few times a day. After the crash, the whole panel disappears, then
reappears and appears to behave normally.
