I'm not sure if this is really the issue you're seeing, but I found something
that can cause colors to not be added to the history.
The popup palette seems to be the home of several ugly hacks, one even relies
on patches in ext_qt as I recently found out.
If it only happens when you make a stroke quickly after closing the palette,
you're probably seeing the effect of this commit:

I honestly can't follow which issue this is supposed to fix, but basically it
blocks the update of recently used colors for 0.75s (plus event loop latency, I
assume) after closing the popup palette. Given that it's almost 10 years old,
chances are the causes have been refactored at least once and it needs
reevaluation.
