Excellent point on Firefox: I think I just assumed it was a kwin issue separate from the underlying programs, but it does appear to be related to Firefox/Mozilla. I tested a variety of different programs with various toolkits (Qt5, Qt4, Qt3, Motif, gtk2) and was only able to reproduce it with Firefox and Thunderbird (but not Seamonkey).
