I just wanted to throw in my two cents here. i just had this issue on KDE Neon
5.12.0
After changing icon themes back to Breeze, and removing icons from my
Icons-only Task Manager, then re-pinning applications to the Task Manager and
re-applying my Papirus icon theme, it seems to be OK for now.
I'll keep an eye out and comment again with more info if I see it again in the
next few days.
