My games are all jumbled up. I just deleted all the empty folders from the default subcategories, but it didn't change anything. The problem persists, and it's indeed randomly the first icon or the first row of icons that are duplicated in the wrong category. I've always experienced this with the Games category, and my game icons are very customized but limited to 256x256px. However, I just tried it with the Utilities category, where there are many programs, and some icons are also duplicated in other categories (for example, the first two).
