This isn't actually a digiKam bug. Some of it also depends on the rotation
settings in digiKam. Please take a screenshot of your rotation settings in
digiKam.

Your HEIF sample is only rotated using Exif metadata. By default, digiKam
respects Exif rotation. Other graphics programs may not be able to do this, or
Exif rotation may need to be enabled.
To ensure that users see everything correctly when sharing images, images
should be rotated using pixel rotation and the Exif metadata flag should be set
back to "normal." digiKam can do all of this via the rotation settings. Note
that pixel rotation can lead to image loss, depending on the format. However,
digiKam performs lossless rotation for JPG images, for example, if the image
allows it.
