I do not have any duplicate entries.
What I do have is:
- entries with missing icons
- missing entries
e.g. Okular was installed just recently and seemingly was not added to the list
of apps. When I opened Okular, it showed a list of recent files, even though my
settings were always set to "Do not remember".
