I have similar problem with 5.4.3 version on OpenSUSE Leap 42.1.
When icon to the new submenu is added, it has no effect for menu - there is
still some ugly default icon visible. Appropriate file is not changed in
~/.local/share/desktop-directories .
There is Icon=package forever.... in *.directory files.
Logout/Login does not help - of course.
