Summary: standardize raft quorum state transitions

Standardize what transitionToX, onBecomeX accomplish. Currently it is not
consistent nor clear which method is responsible for doing what (e.g. checking
if transition is legal, doing in-memory and durable transition, resetting
connections, etc.)
