Move anchor location in changes.xml to header

When navigating to one of the anchor links for a particular version on the
page, the browser will take you past the header and directly to the table under
the header such that the header is no longer visible. It would be a little bit
better if the anchor link was tied to the header so that it's immediately clear
that the navigation took you to the correct set of changes without needing to
scroll back up the page.
