That issue was always present in doxia-converter. Just look at the output
generated from a doxia file containing the TOC macro with an earlier version
and with latest master. Earlier the TOC macro was no longer present in the
destination doxia file (but just actual links and texts for the toc), now the
TOC macro is kept.
