   I wanted the intro to define what an MDV is, the same way the Deletion 
Vectors section defines DVs. I reworded this to say MDVs identify entries that 
are no longer live, but I kept that a set bit means the entry was either 
deleted or replaced since that's still true.

   I kept this as should. The must part is already covered by the MDV delete 
and replace rules. Making it if and only if would mean writers *must* clear 
these in every later commit, and like you mentioned they don't necessarily need 
to be dropped, though we should encourage that, hence the "should" statement. 
They also stay accurate if they're carried over since they're tied to 
`modified_snapshot_id`.
