Removing a wrapper set from itself can throw `ConcurrentModificationException` after removing only part of its contents. Handle self-removal by clearing the set and reporting whether it changed, without changing how removal from other collections compares wrapped values. Test plan: Exercise self-removal for empty, singleton, and multi-element sets, checking the contents and return value. The multi-element case reproduces the exception before the fix.
