Summary: Partitioned Indexes need to remove itself from the index
list correctly

Partitioned Indexes are not properly removing themselves if they fail to get
created. This leads to possibly inconsistent indexes, incorrect results and
possibly confusing error messages. This may also lead to the inability to
create an index that shares the index expression of the failed index.
