With the tls_ca_file line removed, Thunderbird asked me to specify a client certificate, I chose my cert and entered my password to access it.
It's quite possible that authentication with a certificate is broken. I helped fix a few SSL-related bugs that affected us, but we don't use certificates. That means that the changes that were made weren't tested using such a setup.
