SASL version is 2.1.15, too. Neither syslog nor running saslauthd with debugging activated brought any new hints :-/
Does saslauthd dump when you login with an invalid username?
The only other thing I can suggest is to upgrade sasl first. You should also upgrade cyrus-imap; it is kinda old...
