Make sure you also take a look at Mailscanner; at least with sendmail it is quite easy and transparent to get running, and might scale better than a milter solution. It also provides the ability to virus scan at the same time:

The next piece that I'd like to play around with is integrating spamassassin (SA) in to identify the spam that does get through.
