Interesting.  I tested with the info command and I get the expected behavior 
using:

info --file=./sample.info

Here are the steps to reproduce it in Emacs:

1. Run emacs -q
2. Drag sample.info into the Emacs frame
3. M-x Info-mode
4. M-x rev-b, y
5. Select 'Index'
6. Select 'the spot'

It takes me to Top.
