I haven't checked this with me, ms, and mm, but given that the situation does occur more frequently than one would like, I suspect that anyone writing a macro package that supports footnotes will feel compelled to implement such handling. My own macros do it, and there's even a basic implementation in the troff tutorial notes in the user's manual.
